We have an opportunity to participate in meetings and in this forum, we discuss any changes with service users, and this build our confidence for practice.

Our Response Time:
We aim to respond within a maximum of 2 hours with capacity answers and preferred call times and package of care start date.
We aim to carry out our assessments within 24 hour of receiving and triaging your request/ or referral

We work alongside district nurses and other professionals involved in your journey, as we continue to make you as comfortable as possible. We will assess your needs and we will be able to communicate effectively for you if not able and we support you with:

We provide you with Short term care following hospital stay.
Following any of these hospital discharges our front-line staff can assist you with:
Participation in rehabilitation exercises.

(a) a short-term episode of care to enable you regain and retain independence and facilitate your rehabilitation.
(b) Long term care to ensure you continue to live at home independently without having to go into residential care when it is not necessary.
